I don't think its a big deal, if the dates don't match up, because I think both dates are valid...one is the date I ordained them and the other would be the date ULC recorded them into the church rolls....but it would be nice and help to avoid confusion if both dates could be lined up. Consider filling in the date after they have heard from U.L.C.That way the dates will match and you can still have something official to give them when you preform the service.
