Be sure to include your contact information so that we can confirm this change or clarify any questions. Only legal names should be included in your ordination record (not your spiritual name, if different. If your name has changed you must contact us directly, as we will need to obtain additional information as to the circumstances and may require you to furnish documentation of the name change. Do not submit name changes here.

Please include as much information as possible regarding your previous and current address so that we can locate your record. If you do not know the date of your ordination, include your best estimate, and check the box.

Address updates are not done automatically by computer, so these changes are not reflected instantly. If you will be placing an order, please include your old and new address information with your order rather than doing it here. Include your old address in the special instructions (at checkout) and we will update your address when the order is processed.
